Based screening reduces deaths from prostate cancer, finds new review

Men who get a controversial blood test that looks for signs of prostate cancer appear to have a reduced risk of death from the malignancy, according to a new analysis by an worldwide group of researchers. All of these participants were divided into intervention and control groups.

Etzioni was part of a team from the University of MI and the National Cancer Institute that used a new mathematical model to re-analyze data from two major clinical trials: the European Randomized Study of Screening for Prostate Cancer (ERSPC) and the US -based Prostate, Lung, Colorectal and Ovarian Cancer Screening Trial (PLCO), which offered conflicting results.

However, these symptoms can have many other causes than prostate cancer, such as a benign enlarged prostate.

In his statement, the president said there are reasons to be hopeful about finding a cure for prostate cancer as the rate of new cases in the US has fallen almost 6 percent on average every year over the past decade. The average age at the time of diagnosis is about 66.

Etzioni said the new results don't mean all men should be screened for prostate cancer.

But a change in a man's reading on the PSA test is a highly imperfect gauge of trouble: approximately 80 percent of positive PSA test results are thought to produce false-positives, creating scares that prompt men to get biopsies. The European Randomized Study of Screening for Prostate Cancer (ERSPC) which included men 55 to 69 years, however, found that PSA screening led to a 21% lower risk of dying from prostate cancer during the study follow-up compared to men who didn't get screened.

The team of investigators from the University of MI and the National Cancer Institute sought to formally test whether the effects of screening on prostate cancer mortality differed between the ERSPC and PLCO and to estimate the effects of screening in both trials relative to no screening. The study was funded by the National Cancer Institute. Patients with a family history of prostate cancer are at the highest risk for contracting the disease. Individuals of Afro-Caribbean or African descent are at a greater risk of developing the cancer, while those of Asian descent are at less risk.

In 2009 a New England Journal of Medicine editorialist superlatively called the debate over PSA testing for prostate cancer as a dispute that refused to die down. Prostate cancer remains the second-leading cause of cancer death among men in the US and is most successfully treated when detected in its early stages. This is called "watchful waiting" or "active surveillance" where the patient is monitored for cancer progression. But once prostate cancer begins to grow quickly or spreads outside the prostate, it is risky. Side effects from treatment of prostate cancer with surgery or radiation therapy can include urinary incontinence, erectile dysfunction, and bowel problems.
